    What Is IRMAA

    Income-Related Monthly Adjustment Amount. Higher-income Medicare beneficiaries pay extra premium on Part B (medical) and Part D (drugs). Based on income from 2 years prior.

    2025 IRMAA based on 2023 income. Big retirement-year income (Roth conversion, capital gains, business sale) creates 2-year-later IRMAA spike. Plan ahead.

    Avoiding IRMAA

    Roth conversions before age 63 (no IRMAA impact until age 65+2). Long-term capital gains harvesting before Medicare. Charitable giving via QCDs from IRA reduces RMDs and thus MAGI.

    Appealing IRMAA

    If income drops due to life-changing event (retirement, divorce, spouse death), file SSA-44 to appeal IRMAA. Common appeals win for retirement year transitions.

    How much can IRMAA add to my Medicare costs?

    Up to $443.90 extra Part B and $85.80 extra Part D per month, over $6,300 annual surcharge per person at the highest tier.

    When do IRMAA brackets reset?

    Annually, with cost-of-living adjustments. Most years brackets adjust slightly upward.

    Does IRA distribution count?

    Yes, Traditional IRA, 401(k) distributions count toward MAGI. Roth distributions don't (already taxed).

    Can married couples reduce IRMAA by filing separately?

    Sometimes. MFS uses different (lower) brackets but loses many other deductions. Calculate both ways.

    How is IRMAA collected?

    Deducted from Social Security check, or billed quarterly if no SS yet. Cannot be paid pre-tax.
